Important Forms:

  • New Patient Information Packet: This packet should be completed and returned to our office for patients who would like for MOFMC to begin serving as their Primary Care Provider. This is for patients that are transferring their care from another clinic to MOFMC.
  • Walk-in Patient Information Packet: This is for patients who have primary care at another office and do not want to transfer their medical records and care to MOFMC, but only want to utilize the MOMFC Walk-in Clinic services or be seen in the Concussion Clinic.

Steps to becoming an established patient:

  • Complete the following packet completely.
  • The records release page must be completed in the packet so we know where to request your previous medical records. If there are no medical records for us to request, please indicate this on this page of the packet.
  • Drop off the completed forms at the front desk.
  • We will begin to process your information and request your medical records in the order the packets are received.
  • One of our staff members will call you once we have received your records to schedule an appointment.
